失眠有短期和慢性的,存在很多原因:1.短期失眠:通常是由暂时情况或环境问题引起的。它们可能包括:

- 周围环境的变化

1.褪黑素:
褪黑激素是一种在松果体中产生的激素,与身体的睡眠-觉醒周期高度相关。白天,松果体相对不活跃,但当太阳下山时,它开始产生褪黑激素。在人类中,大脑褪黑素的产生一般在晚上9点达到峰值,这与身体的正常睡眠时间相吻合。褪黑激素水平通常在血液中保持高水平约12小时,到了白天,水平自然下降到几乎无法检测的水平。注意,光照是调节人类睡眠模式的一个主要因素。光照会刺激从眼睛到大脑的神经,发出体温升高和皮质醇等刺激激素释放的信号。相比之下,黑暗会刺激褪黑激素的产生并降低体温1,2。
褪黑激素水平低与失眠有关,尤其是在老年人中。在一项临床综述中,据报道,与年龄匹配的正常对照组相比,患有失眠的老年受试者的血清褪黑素水平显著降低(并且褪黑素峰值的时间延迟)3。
研究表明,补充褪黑激素可以改善睡眠。一项对19项随机安慰剂对照试验(包括1683名患者)进行的荟萃分析发现,褪黑素可以改善睡眠质量,增加总睡眠时间,降低睡眠延迟4,5。其他研究表明,褪黑素可以提高睡眠后的警觉性6,并减少人们在夜间醒来的次数7。
一项随机、双盲、安慰剂对照试验涉及125名患有自闭症谱系障碍的儿童和青少年,他们正在与睡眠障碍作斗争。该试验发现,持续13周的2-5mg缓释褪黑素迷你片增加了总睡眠时间,减少了睡眠潜伏期和睡眠障碍。本试验的受试者试图在试验前仅通过四周父母主导的行为干预来改善他们的睡眠,但没有成功8。
2.色氨酸:
L色氨酸是血清素和褪黑激素的前体9,10。L-色氨酸补充剂可能会增加松果体产生的褪黑激素量,从而促进睡眠11。补充L色氨酰在睡眠领域一直备受关注:早期的研究发现,1g可以减少入睡所需的时间12。与褪黑激素一样,L-色氨酸水平随着年龄的增长而下降11;因此,补充L-色氨酸可能有助于治疗老年失眠。
动物研究发现,补充色氨酸会降低夜间的活动,并导致其他有利于睡眠的生物变化,如核心体温下降和白细胞介素IL-6水平降低13。在一项小型人体临床试验中,静脉输注L-色氨酸导致血浆褪黑素水平显著升高,并具有诱导睡眠的作用,无论白天还是晚上都是如此14。此外,L-色氨酸可能有助于缓解某些形式的抑郁,而抑郁会加重失眠15。
对美国国家健康和营养检查调查(NHANES)(2001-2002)涉及29,000多人的数据进行的分析确定,色氨酸的摄入与睡眠时间呈正相关16。另一项研究调查了35名55-75岁的人食用添加色氨酸强化的谷物是否能改善睡眠/觉醒周期。该研究比较了受试者的日常饮食、每30g谷物富含22.5mg色氨酸的谷物(早餐和晚餐时食用)或每30g谷类富含60美国色氨酸(早餐和晚饭时食用)的摄入量。色氨酸含量较高的谷物可提高睡眠效率和总睡眠时间,减少睡眠碎片和睡眠潜伏期17。
另一项涉及33名男大学生的随机研究调查了早餐时间摄入色氨酸和白天暴露在阳光下对褪黑激素分泌和睡眠的影响。受试者摄入了不含色氨酸且光线昏暗的膳食、富含色氨酸但光线暗淡的膳食、缺乏色氨酸而光线明亮的膳食,或富含色氨酸且阳光照射的膳食。明亮的光线和早餐富含色氨酸的膳食相结合,促进了晚上褪黑激素的分泌,并支持更高质量的睡眠18。
3. 甘氨酸:
甘氨酸是一种在大脑中传递化学信号的非必需氨基酸,有助于支持骨骼健康、消化和代谢19。这种化合物也是促进健康睡眠模式的一种新的安全方法。在一项随机、单盲、交叉试验中,11名健康志愿者在睡前一小时服用3g甘氨酸,入睡时间缩短,睡眠满意度提高,第二天没有嗜睡20。在一项随机化、双盲、交叉实验中,睡前3g甘氨酸改善了睡眠质量和第二天疲劳21。在一项随机、单盲、交叉试验中, 10名睡眠受限但其他方面健康的男性在睡前服用3g甘氨酸或安慰剂。第二天,参与者进行了记忆力、认知能力和反应时间的测试,并对他们的疲劳程度进行了问卷调查。与对照组相比,服用甘氨酸的受试者减少了疲劳和嗜睡,并改善了神经行为表现22。
在动物模型中,给睡眠受到干扰的大鼠服用甘氨酸会缩短睡眠潜伏期,降低体温,并诱导非快速眼动睡眠23。口服甘氨酸也可能有助于提高血清素水平,血清素是一种在情绪、学习、记忆、食欲和睡眠中发挥作用的神经递质。
4. 镁:
镁在细胞通讯、能量生产、肌肉功能、血糖维持和昼夜节律调节中发挥作用24。美国国立卫生研究院膳食补充剂办公室(NIH.ODS)建议31岁及以上的男性每天摄入420mg,女性每天摄入320mg25。
镁的最佳水平与正常的睡眠调节有关26。镁缺乏与睡眠时间缩短有关27,并可能引起炎症,从而导致与睡眠不良有关的疾病,包括糖尿病和心血管疾病28。此外,研究表明,随着睡眠限制的增加,细胞内镁浓度下降29。补充镁、褪黑素和锌已被证明可以改善老年人的睡眠30。

苏糖酸镁可能对睡眠有益,因为它比其他形式的镁更有效地穿透血脑屏障,提高大脑镁水平32,33。
5. 锌:
锌对许多生物功能至关重要,包括免疫健康和神经认知,可能有助于降低炎症性疾病的风险34。研究表明,锌也可能在促进睡眠方面起作用30。二项文献综述表明,血清锌水平随睡眠时间的长短而变化,口服锌可以增加睡眠时间和睡眠质量35,36。在另一项试验中,喂食含有锌的酵母提取物的老鼠在睡眠时增加了非快速眼动睡眠,减少了睡眠时运动活动水平37。

6. 缬草:

在一项涉及100名患有失眠的绝经后妇女的随机、三盲对照试验中,用530mg缬草根治疗四周可改善睡眠质量43。一个临床前模型研究了啤酒花和缬草混合物对睡眠质量的影响。研究发现,治疗组的睡眠行为包括总睡眠时间,通过调节GABA信号通路得到改善44。
一项研究比较了600mg缬草与常用镇静剂奥西泮的效果。在六周的治疗过程中,缬草显示出与10mg奥西泮相当的疗效45。相关研究综述表明,缬草是睡眠问题患者的一种可取治疗选择,而且它似乎具有良好的安全性,与常见药物比没有已知的相互作用46。缬草的典型剂量约为300-600mg,入睡前30至120分钟服用47。缬草的镇静作用可能需要每天使用到两周才能完全显现48。
缬草在局部使用时也可能有用。一项涉及85名重症患者的随机临床试验发现,在晚上7:00至10:00之间用缬草穴位按压会增加睡眠时间,减少夜间醒来的频率49。
7. 西番莲:
西番莲在欧洲长期以来一直被用于治疗焦虑和失眠50。西番莲中的活性化合物似乎与GABA和阿片类药物系统相互作用51-53。这种植物的提取物在临床前研究中显示出镇静、抗糖尿病和抗哮喘的活性50。
在一项随机双盲试验中,78名失眠症患者接受了10mg剂量的唑吡坦或啤酒花、缬草和西番莲的组合治疗,为期两周,两组的睡眠时间、睡眠潜伏期和夜间清醒情况都有所改善54。一项随机研究比较了59名患有睡眠障碍和抑郁症的更年期妇女服用西番莲或圣约翰草的效果。六周后,两组的睡眠质量都有所改善55。在一个动物模型中,西番莲被证明可以减少焦虑行为52。此外,另一个动物模型发现,当在三周内给药时,西番莲衍生的化合物可以防止小鼠对地西泮的依赖56。虽然这种提取物具有良好的安全性,需要更严格的临床数据来评估其在人类中的疗效。
8. 香蜂草:

9. 洋甘菊:
洋甘菊是一种受欢迎的草药,经常用来泡茶以促进睡眠和放松65,66。洋甘菊的干花提取物含有萜类和黄酮类化合物,可以帮助对抗炎症、失眠、溃疡、伤口、疼痛和肌肉痉挛。这种草药的镇静作用被认为是由于某些黄酮类可调节大脑中的苯二氮卓类受体信号传导67。

10. 薰衣草:
精油芳香疗法是一种利用植物油治疗健康问题的替代医学实践。薰衣草油已被广泛研究用于治疗失眠。一项涉及79名睡眠困难大学生的随机对照临床试验发现,吸入薰衣草与良好的睡眠卫生习惯相结合,在改善睡眠质量方面比单独使用睡眠卫生更有效72。在另一项试验中,67名患有失眠的中年妇女被随机分组,每周两次吸入薰衣草油20分钟,持续12周,或接受睡眠卫生教育。与对照组相比,治疗组的睡眠质量有所改善73。
在一项为期四周的研究中,28名产后妇女与焦虑和抑郁作斗争,受试者要么被随机分为一组,吸入玫瑰奥托(Otto)和薰衣草精油的混合物,要么被指示在研究期间避免使用精油。芳香疗法组在抑郁和焦虑评分方面有显著改善,负面影响的风险最小74。研究发现,薰衣草油还可以减轻醒来后的嗜睡感75。在动物模型中,七天使用薰衣草油可以减少与抑郁和焦虑相关的行为。该精油疗法也被证明可以减少攻击行为,改善社会互动76。

11. 藏红花:

12. 厚朴和黄柏:
和厚朴酚(Honokiol)是主要从厚朴树的树皮中提取的一种配体。传统上,它被用于治疗情绪障碍,包括焦虑和抑郁83,并且被认为具有抗肿瘤、抗炎和抗氧化作用84。它可以跨越血脑屏障,从而提高生物利用度,研究表明,和厚朴酚对中枢神经系统具有神经保护作用85。和厚朴酚可增强海马神经元中GABA受体的活性83。

自我管理
- 睡眠习惯会影响睡眠质量的好坏。有助于良好睡眠的措施包括如下:
- 起床和睡觉,每天在同一时间。

- 只在卧室里睡觉。
- 保持卧室舒适的温度和无光线。尽量减少干扰,比如宠物。
